Does Root Canal Treatment Hurt?

Does root canal treatment hurt is the question I am asked more than any other, usually with real apprehension behind it. The honest answer is no. Root canal treatment is carried out under thorough local anaesthetic, and most patients describe it afterwards as feeling much like having a long filling.

The reputation belongs to a different era of dentistry, and it deserves correcting, because fear of this treatment keeps people in genuine toothache for weeks unnecessarily.

The honest answer

Root canal treatment relieves pain rather than causing it. That is the fundamental thing people have backwards.

The pain associated with root canals is the toothache that leads people to need one: an infected or dying nerve inside the tooth, which is genuinely one of the more unpleasant pains there is. The treatment removes that nerve tissue, which is precisely what makes the pain stop.

During treatment, the tooth and surrounding area are completely numbed, and numbness is tested and confirmed before anything begins. You should feel pressure and movement, but not pain.

Where the reputation came from

Two things, both historical.

Decades ago, anaesthetic techniques, instruments and materials were considerably less advanced, and treatment was slower and less comfortable. That generation’s stories have been passed down, and they have outlived the reality by a long way.

The second reason is more subtle. People often arrive for root canal treatment already in severe pain, sometimes having endured it for days. The association between “root canal” and “agony” is real, but the agony came first and the treatment ended it.

What it actually feels like

Practically, here is the experience:

  1. Numbing. A local anaesthetic injection, no different from having a filling. This is usually the only part you feel.
  2. Waiting a few minutes, and I will check the tooth is properly numb before starting. Extra time and care is taken here if the tooth is inflamed, because inflamed tissue can be harder to numb.
  3. Access. A small opening is made in the tooth, which feels much like a filling being prepared.
  4. Cleaning the canals. Fine instruments clean the root canals. You feel movement and hear the instruments, but no pain.
  5. Sealing. The canals are filled and sealed.

The main challenge most patients report is keeping their mouth open for a while, rather than any discomfort. Appointments typically run around an hour, sometimes longer for back teeth with several canals, and treatment is completed in one or two visits depending on the infection.

How you feel afterwards

Some tenderness for a few days is normal as the surrounding tissues settle, particularly when biting on that tooth. Ordinary painkillers taken as the packet directs handle it comfortably for most people.

What patients most often report is relief: the deep, throbbing toothache that brought them in has gone.

Afterwards the tooth is restored, and back teeth usually need a crown to protect them, because root-treated teeth become more brittle over time. Our post on how long crowns last covers that stage.

What if I am already in pain?

Then getting treated sooner rather than later is very much in your interests, and pain is not a reason to delay.

Where a tooth is acutely inflamed, extra care is taken with the numbing, and additional techniques are available if a tooth proves stubborn to anaesthetise. Occasionally antibiotics are used first to settle an acute infection so treatment can be carried out comfortably, though as our post on whether antibiotics cure tooth infections explains, they settle rather than solve.

The alternative is worse

Worth stating plainly: leaving an infected tooth untreated does not resolve it. The infection progresses, often to an abscess, which is considerably more painful than the treatment being avoided, and it can damage the bone around the tooth to the point where the tooth becomes unsaveable.

The realistic choice is not between root canal treatment and nothing. It is between root canal treatment and losing the tooth, and our page on tooth extractions explains what the second road involves.

If you are genuinely frightened

Say so when you book, and it changes how the appointment is run. Everything is explained before it happens, you can agree a signal to pause at any point, and nothing starts until you are completely numb and ready.

What to take away

Root canal treatment does not hurt. It is carried out under thorough anaesthetic, feels much like a long filling, and removes the toothache that made it necessary. A few days of mild tenderness afterwards is normal and settles quickly.

Dr Bilverdi is absolutely fantastic - best dentist I've ever been to. He's extremely accomodating, patient and caring, and has done a fantastic job on my teeth (fillings and a crown). He always goes the extra mile and really cares about his patients. The prices are very reasonable as well. Hope, the dental nurse is also just so lovely. I have been a patient here for a few years now, and have always had an excellent experience! My young children attend with me, they are treated with kindness and sincerity and have no concerns about visiting the dentist. Always super clean, all of the equipment, as well as the surgery itself are maintained to a very high standard. Dr Bilverdi and Hope are clearly very experienced and provide a great service.

As a very nervous patient (we're talking panic attack in the waiting room in the past) Dr. Bilverdi and his nurse have been exceptional. I'm no longer scared to go to the dentist! He has a very calming and reassuring manner and takes the time to explain everything clearly. A single-dentist practice that has been there for 26 years, with a nurse who has worked with him for 15 years, is the main reason I chose this practice. That's not to say the other practices in the area aren't good. But I found Dr Bilverdi to be outstanding in all areas. As an anxious patient, it's good to know who I'm seeing every time. It's refreshing when you call to make an appointment, and the dentist answers the phone, and then when you arrive, he is sitting at the reception desk to greet you. Had to have several injections today and can honestly say that I barely felt a thing with zero pain! The numbing spray before the injections worked brilliantly. If you are nervous about visiting a dentist, I can highly recommend Dr Bilverdi at Tender Dental Care.

I have recently had a cracked tooth, i got an appointment, i am so fearful of the dentist but when i got there such a lovely reception, the receptionist was absolutly brilliant, made me feel so welcome, and the dental nurse made feel that i had nothing to worry about, especially when u are so fearful of a dentist, and the clinician that actually did the work for me was so welcoming and again made me feel that i had nothing to worry about so professional, all of the team, thankyou so much. I have been seeing Dr Bilverdi for nearly 20 years now. His care and attention to detail is first class and I have never had any trouble with his work. No filling or crown has fallen out and no treatment has had to be repeated. If you are not cleaning any areas he will point this out. A top class dentist and his assistant Hope is always very helpful and efficient.

I’m terrified of the dentist! This has meant that I have avoided attending pretty much all my adult life. My teeth were in an awful condition and I had to do something. I rung around several dentists before settling on Tender Dental Care. They could see me weeks ahead of any other practice but what sold it to me was the fact that the dentist himself answered the phone and was really friendly. I was so nervous about going and actually quite embarrassed about the state of my teeth but Dr. Bilverdi was not only patient but totally non-judgemental. He explained everything that was happening and didn’t go straight in for “you need lots of fillings”, which is always a concern when you pay privately. He instead gave me a whole range of options to consider in my own time. Dr. Bilverdi really took his time with me and at no point did I feel rushed or like I couldn’t ask more questions. Hope is also lovely and the quiet, nicely decorated environment is so much nicer than the usual bustling dentist waiting room. I was the only person on site during the appointment. The price is pretty reasonable too, with each cost being broken down before you agree to it. I have finally found a dentist I trust and will absolutely not be leaving it 6 years until my next visit! I went in terrified and tearful that I was going to loose all my teeth but left feeling relieved and relaxed. I can highly recommend!

I contacted this dentist for an emergency appointment today due to severe pain. I was frightened due to a previous experience at another dentist.Hope the dental nurse and Mr Bilverdi were very good they reassured me and explained what they were going to do throughout my consultation. I needed an urgent appointment because of a tooth infection. I was seen to really quickly, treated with antibiotics. The tooth was taken out a week later. I was really nervous but I was put at ease by Dr Bilverdi and his assistant. The tooth came out OK in fact I didn't even feel a thing. I recommend Tender Dental Care, as the name says, Tender care.

I have now been to Tender Dental Care several times and I am so very pleased with the excellent treatment I have received. Dr Bilverdi has been very kind , gentle and understanding. And his nurse Hope is a very kind young lady too. I have had too many bad experiences with dentists over the years but I am now trusting and at ease when I see Dr Bilverdi. Would highly recommend !
