If you have been told a tooth needs a crown, it is worth asking whether an onlay would do instead. Having an onlay instead of a crown preserves considerably more of your natural tooth, and for a good number of teeth it is the better long-term decision. It does not suit every case, and knowing which is which is the whole of the skill.

The difference in what is removed
A crown covers the entire tooth above the gum line. To fit one, the tooth is reduced all the way round, on every surface, including parts that may be perfectly healthy.
An onlay covers only the damaged parts: the biting surface and whichever cusps need protecting. Sound walls of the tooth are left untouched.
In practical terms, a crown typically involves removing substantially more tooth structure than an onlay for the same tooth. That difference is permanent, since enamel does not grow back.
Why preserving tooth matters
Not for its own sake, but because of what happens over decades.
Teeth tend to be restored several times across a lifetime. Each restoration removes a little more, and the more tooth remains at each stage, the more options are available at the next. A tooth restored conservatively at forty has better prospects at seventy than one crowned unnecessarily at forty.
There is also a nerve consideration. The more tooth removed, the closer the preparation comes to the pulp, and the greater the chance the tooth eventually needs root canal treatment. Conservative preparation reduces that risk.
When an onlay will do
An onlay is often appropriate when:
- The damage is confined to the biting surface and one or two cusps, with the remaining walls sound
- A large old filling has failed, but the tooth still has good structure around it
- A single cusp has fractured, which is the classic onlay case
- The tooth needs bracing against a crack but is otherwise intact
- The tooth is root-treated but has retained substantial structure, where an onlay can sometimes protect it adequately

When a crown is genuinely needed
Equally, sometimes a crown really is the right answer, and settling for less would be false economy:
- Multiple walls of the tooth are gone or undermined, leaving nothing for an onlay to bond to reliably
- The tooth is severely broken down, perhaps with a large post-and-core build-up
- A root-treated back tooth has lost substantial structure, where full coverage gives better protection
- The tooth is cracked in a way that needs complete encirclement to hold it together
- Appearance requires it, particularly on a visible tooth needing significant colour or shape change

Are onlays strong enough?
This is the usual concern, and modern materials have largely answered it.
A bonded ceramic onlay does not simply sit in the tooth; the bonding process means the restoration and the tooth function together as a unit, which reinforces the remaining structure rather than merely occupying space.
Where onlays struggle is when there is too little sound tooth to bond to, or where the bite is exceptionally heavy. Cost and lifespan
Onlays typically cost more than a filling and less than a crown, sitting exactly where they do clinically.
Lifespan is comparable to crowns, commonly ten years or more with good care, and what determines it is the same for both: decay at the margin, grinding, and how well you clean.

How to ask about it
If a crown has been recommended and you would like to explore the alternative, the question to ask is simply: “Is there enough sound tooth left for an onlay, or does this genuinely need full coverage?”
That is a fair question and any dentist should be happy to answer it with the reasoning shown, ideally with the tooth or an X-ray in front of you. If the answer is that a crown is needed, you will understand why. If an onlay would do, you have preserved part of your tooth.
What you should never encounter is irritation at being asked. Informed patients make better decisions, and the General Dental Council’s standards are clear that patients should have the information they need to decide.
The bottom line
An onlay instead of a crown is often possible where the tooth still has sound walls, and it preserves considerably more natural tooth for the future. Where multiple walls are gone, the tooth is severely broken down, or a crack needs full encirclement, a crown is the honest answer. Ask which category your tooth is in, and expect the reasoning.
