How often should you have a scale and polish is a question with no single right answer, which is unsatisfying but true. Many patients do well with one every six months alongside their check-ups. Others need three-monthly visits, and a few can comfortably go longer. What decides it is the state of your gums rather than the calendar.

What a scale and polish actually does
Two jobs, as the name suggests.
The scale removes plaque and hardened tartar from your teeth, above and just below the gum line. Tartar is plaque that has calcified onto the tooth, rather like limescale in a kettle, and once it has set no toothbrush will shift it. It needs professional instruments, and it matters because tartar is a rough surface that gathers yet more plaque, inflaming the gum next to it.
The polish smooths the tooth surfaces and lifts everyday surface staining from tea, coffee and red wine. That smoothness is not just cosmetic: plaque finds it harder to cling to a polished surface, so the clean keeps working for weeks after you leave.
How often should you have a scale and polish?
The honest starting point is that your interval should be set for you rather than by a rule. For most patients with reasonably healthy gums, every six months works well and pairs neatly with a check-up.
Your gums are measured at your examinations, so the decision is based on evidence rather than guesswork. If the numbers are good and staying good, your interval can stretch. If they are drifting, more frequent visits catch it early.
Who needs them more often
Three-monthly or four-monthly hygiene visits are commonly recommended for:
- Anyone with a history of gum disease. Once gum disease has been treated, it is controlled rather than cured, and regular professional cleaning is what keeps it controlled.
- Smokers. Smoking accelerates gum disease and masks the bleeding that would otherwise warn you, so closer monitoring matters more.
- People who build tartar quickly. This varies genuinely between individuals and is partly down to saliva chemistry rather than effort.
- Anyone with diabetes. The link between gum disease and blood sugar control runs both ways.
- People with lots of dental work, such as bridges, crowns or implants, where cleaning is more demanding.
- Patients in orthodontic treatment, where appliances make plaque control harder.
Who can go longer
If your gums are consistently healthy, you clean thoroughly between your teeth every day, you have little dental work, and you do not smoke, an annual hygiene visit alongside your check-up may well be enough.
The key word is consistently. This is a judgement made from your gum measurements over time, not from how well you brushed in the week before your appointment, which, if I may say so gently, is a pattern I am familiar with.
What happens if you skip them
Tartar accumulates, and the gum next to it becomes inflamed. That is gingivitis: red, puffy gums that bleed when you brush. At this stage everything is fully reversible, and a good clean plus improved home care resolves it within weeks.
Left longer, inflammation spreads below the gum line, pockets form, and the bone supporting the teeth begins to be lost. That is periodontitis, and the difference matters enormously: bone that has gone does not come back. Gum disease can be halted at that stage, but not undone.
Since gum disease is the most common cause of tooth loss in adults, and it rarely hurts until it is advanced, regular cleaning is genuinely one of the better investments in dentistry. Our page on gum disease treatment explains the stages in more detail.
Does it hurt?
It should not. Most people find a scale and polish perfectly comfortable, with the ultrasonic scaler feeling odd rather than painful.
Two things make it less comfortable: sensitive teeth, and inflamed gums. Both can be worked around, and the second improves as your gum health does, so the appointments genuinely get easier the more regularly you attend. Do say if you are sensitive, and the technique will be adapted.

Your gums decide, and the measurements taken at your check-ups are what tell us. Whatever your interval, daily cleaning between your teeth does more for your gums than any appointment can.
