The adverts are persuasive: straighter teeth for a fraction of the price, no appointments, everything posted to your door. Patients ask me regularly whether mail-order aligners are safe, and the honest answer is that the aligners themselves are not the problem. What is missing is everything that happens around them.

How mail-order aligners work
The model is straightforward. You take impressions of your own teeth using a posted kit, or attend a scanning shop briefly. A treatment plan is produced remotely, a full series of aligners is manufactured, and the whole set arrives at your home. You then work through them yourself, usually with app-based check-ins rather than appointments.
The appeal is obvious: cheaper, quicker to start, and no time off work. For a small number of very straightforward cases, people do get results they are pleased with.
What is missing from the process
Here is the part the adverts do not dwell on. Before any orthodontic treatment, a dentist would normally check several things in person:
- Whether you have decay. Moving teeth with untreated decay lets the problem progress hidden beneath an aligner.
- Whether your gums are healthy. This is the important one. Moving teeth in gums affected by gum disease can accelerate bone loss and, in serious cases, loosen teeth.
- Whether your bite works. Straight-looking front teeth with a bite that no longer meets properly is a poor trade.
- Whether the roots are sound, which needs X-rays that a posted impression kit cannot provide.
- Whether aligners suit your case at all. Some movements genuinely need fixed braces.
An impression kit taken at your kitchen table captures the shape of your teeth. It captures none of the above.
What can actually go wrong
Problems reported with remote aligner treatment tend to fall into a few categories: teeth that move in ways that were not planned, bites that no longer meet correctly, gum problems worsened by movement, root damage, and treatment that simply stops short of the promised result.
The common thread is that nobody was watching. With supervised treatment, a dentist sees you periodically and can adjust course when something is not tracking as planned. Remote treatment substitutes photographs you take yourself, which is not the same thing, and by the time a problem is visible in a selfie it has usually been happening for a while.
The retainer problem
Whatever moves your teeth, they will drift back without retention. Retainers, usually worn at night, are permanent, and that is true of every orthodontic treatment on earth.
Where mail-order treatment often disappoints is what happens afterwards: retainers may be an extra purchase, replacements need reordering, and there is no dentist checking your retainer still fits properly a year later. A relapsed result is the most common regret I hear.
Where UK regulation stands
Orthodontic treatment is regulated dentistry in the UK, and the professionals providing it must be registered with the General Dental Council. Professional bodies have raised concerns about direct-to-consumer models specifically because of the absence of an in-person examination before treatment begins.
My practical advice is simple: whoever treats you, you should be able to name the registered dentist responsible for your care, and check them on the GDC register.
Questions worth asking any provider
- Will a dentist examine my mouth in person before treatment starts?
- Will X-rays be taken?
- Who is the named registered clinician responsible for my treatment?
- What happens if my teeth do not move as planned?
- Are retainers included, and what happens when they wear out?
- What is the total cost including retainers and any refinements?
Any provider, including us, should answer all six without hesitation.

No lectures. Plenty of people have, and most come to no harm. What I would suggest is booking a check-up so that your teeth, gums and bite can be examined properly alongside your treatment. If everything is healthy, you carry on with reassurance. If something needs attention, far better to know now.

Mail-order aligners are not inherently unsafe, but skipping the examination that should precede orthodontic treatment is. The risk is not the plastic; it is treating teeth without knowing whether the gums, roots and bite can take it.
